Please provide ALL information required to your team representative so that a pass can be issued in a timely manner. You cannot play without a pass.
The registration kits require:
- TWO copies of a completed and signed registration form.
- One color wallet head shot photo with the player's name written on the back of the photo
- One photocopy of a photo ID (i.e., school ID, drivers license, state issued ID)
- $30.00 registration fee (Your team may charge more for team dues, but your team rep must submit $30.00 for each player registered.
